Major Medical and Dental Insurance
PVMED does not offer medical or dental health insurance to hourly employees.
PERSONAL LEAVE
Full time (40+ hours per week) salaried and hourly employees are can accumulate paid personal leave. Leave will not be granted (but can accumulate) in the first 6 months of employment. Leave requests should be submitted in writing to your direct supervisor four weeks in advance of desired time off. Leave will be approved or denied at the discretion of the PVMED president.
Exempt full-time employees accrue personal leave at the same rate as hourly and non-exempt employees.
Accrued personal leave hours roll over to each new calendar year. The maximum accrued hours of leave an exempt employee may accumulate is 240 hours (that is 6 working weeks, or 30 days). Any hours accrued after 240 hours are lost. Upon employee termination or resignation, unused vacation hours will not be cashed out, and are “lost”.
FAMILY MEDICAL LEAVE ACT
Employees are NOT eligible to request job-protected unpaid leave under the Family and Medical Leave Act of 1993. This act applies to a company of 50 employees or greater in size, and PVMED does not qualify. The granting of leave without pay due to circumstances covered under this act, may be given at the discretion of the PVMED president.

Holidays falling on Saturday are observed on the preceding Friday; those falling on Sunday are observed the following Monday. Only full time employees on full-time paid status (not on leave without pay) will be paid a full day’s wage for holidays. Part-time and temporary employees will not be paid for holidays.
SICK LEAVE
PVMED employees do not earn or accrue paid sick leave. Personal leave may be used instead, at the employee’s request. An employee who does not report to work due to illness, may be placed on paid or unpaid personal leave at the discretion of the PVMED president. For example, unpaid personal leave may be granted for family emergencies involving IRS guideline dependents, after available personal leave has been used.
RETIREMENT PROGRAM
PVMED offers the option of an IRS Simple Individual Retirement Account (SIMPLE-IRA). Under this program, you may elect to automatically set aside a portion of your pay each pay period, and have that amount invested in one of a group of selected investments. Any savings contributions you make to your account are tax deferred, as are any earnings on that money. PVMED will match your contributions up to 3% of your annual salary. (PVMED elects the matching contribution amount each year. This rate may increase or decrease at the Company's discretion. Staff members will receive notification of any changes to the matching funds amount.) Full-time staff members become eligible to participate in this plan after their first 30 days of employment at PVMED. Staff members who opt not to join the program at that time may sign up later at one of two enrollment periods during the year, December or June. Changes may be made to existing plans at those times as well. The details of the plan are contained in a separate package available from the Human Resources department. It is the staff member's responsibility to monitor their plan and make changes to their account as needed.
